Gerald R. Hale, D.O., FAOCA
Dr. Hale is a native Oklahoman and has been a physician for over 22 years, with the last 16 here in Tulsa. Dr. Hale obtained a Bachelor of Science in Pharmacy degree at Southwestern Oklahoma State University. He went on to obtain a Doctorate in Osteopathic Medicine degree at the Oklahoma College of Osteopathic Medicine and Surgery in Tulsa, Oklahoma and served as Class President the final two years. He remains active in the school as an admissions interviewer and alumni. He participates with both OSU and OU providing training for Anesthesia Residents and Sports Medicine Fellows in the area of pain medicine. He is Board Certified in two areas, Anesthesiology and Pain Management and is a clinical examiner for the American Osteopathic College of Anesthesiology and Residency program inspector for the American Osteopathic Association. Dr. Hale is a Board Member for the American Osteopathic Board of Anesthesia and a Fellow of the Osteopathic College of Anesthesiology.
Dr. Hale and his wife Janie live in Tulsa and have two children and one grandchild. Their enjoyments are spending time at the lake, flying, golf, scuba diving, OU football and most important, their grandson.
Annette Stephens, M.D.
Dr. Stephens is a graduate of the United States Air Force Academy and spent nearly ten years as a member of the Air Force. She then went on to medical school at the University of Texas at Houston and anesthesia residency at Emory University in Atlanta, Georgia. Dr. Stephens later completed a pain fellowship through Sloan Kettering Cancer Center and its affiliated programs at New York Presbyterian Hospital/Weill Cornell and Hospital for Special Surgeries (an orthopedic hospital). She is board certified by the American Board of Anesthesiology in both Anesthesia as well as Pain, and is also credentialed by the American Board of Pain Medicine.
Dr. Stephens treats a wide range of painful medical conditions with multimodal therapies, including interventional procedures, in order to limit the need for addictive substances and get patients on the road to recovery. All injections are provided with light or heavier sedation to improve and assist patient tolerance to treatment.
Apart from medicine, Dr. Stephens enjoys international travel and a wide range of outdoor activities like bicycling, hiking and a new interest in sailing.
Kelly Hendricks Volk, D.O.
Dr. Volk attended medical school at A.T. Still University and proceeded to complete residency in the field of physical medicine and rehabilitation at the University of Kansas. Following this, she pursued her passion and continued for a year of fellowship training in interventional spine/ pain management. Her primary goal is to help patients improve their quality of life and decrease pain levels using conservative modalities including physical therapy, medications, and interventional treatments. She is board certified in Physical Medicine and Rehabilitation. Kelly has focused experience in treating a wide variety of musculoskeletal conditions.
